Fosun Health Gynecological Cancer Treatment Center mainly targets cervical cancer, endometrial cancer, ovarian cancer and other female reproductive system tumors, and provides multi-disciplinary diagnosis and treatment relying on gynecology, medical oncology, radiotherapy, imaging, pathology, genetic counseling and rehabilitation. The team will combine imaging, pathology, staging, physical condition and fertility needs to determine the main path of surgery, radiotherapy or systemic treatment.
For patients who are suitable for surgery, the center can perform open, laparoscopic, hysteroscopic or robot-assisted surgery according to the indications, and pay attention to surgical staging, lymph node assessment and postoperative pathological risk stratification. The treatment strategies for cervical cancer, endometrial cancer and ovarian cancer are different. Ovarian cancer usually emphasizes the combination of debulking surgery and platinum-containing treatment, while cervical cancer and some endometrial cancers may require external beam radiation, brachytherapy or drug treatment.
When pelvic radiotherapy is needed, the team can rely on platforms such as VitalBeam to carry out image-guided and intensity-modulated radiotherapy, and manage the consistency of radiation based on bladder filling, intestinal status and changes in lesions, and try to control the dose to the bladder, rectum, small intestine and bone marrow. For advanced or relapsed patients, treatment will also be evaluated based on pathological type, previous medication, and related test results such as MMR/MSI, BRCA, and HRD.
The center incorporates pelvic floor function, urination and defecation, ovarian function, menopausal management, nutrition, thrombosis risk and psychological support into the whole process of management. After treatment, follow-up examinations are arranged according to the cancer type, stage and recurrence risk to help patients make a smooth transition from surgery or radiotherapy to functional rehabilitation and long-term follow-up.
Common treatment pathways for female reproductive system tumors
1. Cervical cancer: Radiotherapy can be used in all stages. More specifically, some early-stage patients can choose surgery or radical radiotherapy; locally advanced disease usually focuses on external beam radiation, brachytherapy and concurrent chemotherapy; patients with recurrence or metastasis need to evaluate systemic treatment and local symptom control.
2. Endometrial cancer: Preoperative radiotherapy is suitable for some larger and poorly differentiated lesions. Postoperative treatment depends on myometrial invasion, differentiation and pathological type. The current approach usually assesses surgical staging first, and then selects observation, vaginal brachytherapy, pelvic radiotherapy, or medical treatment based on stage, lymph node, pathological, and molecular risk.
3. Ovarian malignant tumors: Advanced or residual disease can be treated with local radiotherapy. Ovarian cancer generally still relies on cytoreductive surgery and platinum-containing systemic therapy as the core. After recurrence, drugs, reoperation, or radiotherapy for a few localized lesions are selected based on platinum sensitivity, BRCA/HRD status, and lesion distribution.
4. Malignant trophoblastic tumors, vulvar cancer and vaginal cancer: Emphasize the comprehensive application of chemotherapy, surgery and radiotherapy. Treatment needs to be based on the histological type, lesion extent, lymph nodes and recurrence location, and try to protect the functions of the bladder, rectum, vagina and pelvic floor while controlling the tumor.
